Deployment Health Assessments

Department of Defense Instruction (DoDI) 6490.03 Deployment Health assigns responsibilities for deployment health activities and prescribes procedures for deployment health activities for joint and service-specific deployments in order to monitor, assess, and prevent disease and injury, to control or reduce occupational and environmental health risks, to document and link exposures with deployed personnel, and to record the daily locations of deployed personnel.

The Deployment Health Assessment Program applies to service members, DoD civilian employees and DoD contractor personnel. Procedures related to the administration of the required Pre-Deployment Health Assessment (DD Form 2795), Post-Deployment Health Assessment (DD Form 2796), and Post-Deployment Health Reassessment (DD Form 2900) are detailed in this policy. 

Personnel who are deploying or have deployed must electronically complete the deployment health assessment forms within the required timeframes listed in the chart below. Following completion of the form, a health care provider (i.e., physician, physician assistant, nurse practitioner, advanced practice nurse, independent duty corpsman, independent duty medical technician, or special forces medical sergeant) must review the form, conduct a confidential face-to-face interview, and complete the provider’s section of the form, including indicating the need for further evaluation.

All completed deployment health assessment forms are sent electronically through the customized applications that have been developed by the individual services to the Defense Health Agency’s Armed Forces Health Surveillance Branch (AFHSB), which is the DoD central repository for all deployment health assessment forms. The AFHSB collects and analyzes the forms and posts a monthly summary report entitled Deployment Health Assessments.

The health care provider will place a copy of all completed health assessments in the individual’s permanent medical record.

Notes:

*A PDHRA is required regardless of the number of days after return from theater. After 180 days, the PDHRA is no longer considered in compliance, but a final health assessment is required.

*A final DMHA is required regardless of the number of days since return from deployment. After 910 days, the DMHA is no longer considered in compliance, but a final mental health assessment is required.
