The following DHCC Brochures are available to be ordered:
A maximum number of 50 copies of each brochure can be ordered. A CD with printer-quality brochure content and printing instructions for each brochure is also available to be ordered.
One copy of the following DHCC CDs are available to be ordered:
Orders should be coordinated through the Clinical Practice Guideline Coordinator/Health Care Integrator (HCI) at your Medical Treatment Facility.
CPG Coordinators/HCI should submit their request via email to:

- Name of Military Treatment Facility
- Point of Contact - Name, Rank (if applicable), Phone Number, Email Address
- Title of Brochure(s) and Number of Brochures Requested
- Title of Brochure Printing CD(s) or DHCC CD(s) Requested
A member of the DHCC Staff will contact the POC to confirm the order.
